What Is FUE?

FUE stands for Follicular Unit Extraction. It is a hair transplant technique where individual hair follicles (or “follicular units”) are extracted one at a time from the donor area — typically the back and sides of your head, where hair is genetically resistant to balding — and transplanted to areas where you’re thinning or bald.

Unlike the older FUT (Follicular Unit Transplantation) method, which removes a strip of skin from the donor area, FUE uses a tiny micro-punch tool (0.6–0.9mm in diameter) to extract each follicle individually. This means no linear scar, faster healing, and the ability to wear your hair short afterwards.

How FUE Works: Step by Step

Here’s what happens during a FUE hair transplant procedure:

Step 1: Consultation and hairline design

Before the procedure begins, your surgeon assesses your hair loss pattern, donor hair density, and discusses your goals. Together, you agree on a hairline design and the number of grafts needed. The new hairline is drawn on your scalp with a marker so you can approve it before any work begins.

Step 2: Local anaesthesia

The donor area (back and sides of the head) and the recipient area are numbed with local anaesthesia. You’re fully awake throughout the procedure but feel no pain. The initial injections may cause brief discomfort, but once the anaesthesia takes effect, the rest of the procedure is painless.

Step 3: Follicle extraction

Using a micro-punch tool (often with a sapphire or titanium tip), the surgeon extracts individual follicular units from the donor area. Each unit contains 1–4 hairs. The extracted grafts are placed in a chilled holding solution to keep them viable. A typical session involves extracting 2,000–4,000+ grafts over several hours.

Step 4: Recipient site creation

The surgeon creates tiny incisions (recipient sites) in the balding areas where the grafts will be placed. The angle, direction, and density of these incisions determine how natural the final result looks. This is where surgical artistry matters most. Modern clinics use sapphire blades for finer incisions and faster healing.

Step 5: Graft implantation

Each extracted follicle is carefully placed into a recipient site. Single-hair grafts are typically placed along the hairline for a natural, feathered appearance, while multi-hair grafts go behind to create density. This phase requires precision and patience — it’s the most time-consuming part of the procedure.

Step 6: Post-operative care

Once all grafts are placed, the donor and recipient areas are cleaned. You receive detailed aftercare instructions, medications (antibiotics and anti-inflammatories), and a special aftercare kit. Most patients can leave the clinic the same day.

FUE vs FUT: Key Differences

FUT (Follicular Unit Transplantation) is the older technique, sometimes called the “strip method.” Here’s how they compare:

FUE has largely replaced FUT at modern clinics. The only scenario where FUT may be recommended is when a very large number of grafts is needed in a single session, as FUT can yield slightly more grafts per session. However, most clinics — including those in Albania — now perform FUE exclusively.

Recovery Timeline

FUE recovery is straightforward, but knowing what to expect helps manage your expectations:

  • Days 1–3: Mild swelling and redness. Donor area may feel tender. Sleep with your head elevated. Avoid touching the transplanted area.
  • Days 3–5: First gentle wash at the clinic or following instructions at home. Scabbing begins to form around the grafts.
  • Days 7–10: Scabs fall off naturally. Most swelling subsides. You can return to desk work.
  • Weeks 2–4: Transplanted hairs fall out (this is normal — it’s called “shock loss”). The follicles remain under the skin and will begin producing new hair.
  • Months 3–4: New hair begins growing from the transplanted follicles. Growth starts slowly and may appear fine or wispy at first.
  • Months 6–9: Noticeable improvement in density. Hair thickens and lengthens.
  • Months 12–18: Final results. Full density and thickness achieved. Transplanted hair is permanent.

Results: When to Expect Growth

FUE results develop gradually. The transplanted follicles need time to establish a blood supply in their new location and begin producing hair. Most patients see:

  • 30% of final result at 6 months
  • 60–70% of final result at 9 months
  • 90–100% of final result at 12–18 months

Patience is key. The “ugly duckling phase” between weeks 3 and 12 — when transplanted hairs have shed but new growth hasn’t started — can be discouraging, but it’s completely normal. Every hair transplant patient goes through it.
